By submitting this form, you give VEST permission to record you via audio and/or video during the Zoom and/or any other Live or recorded sessions, use the name and photo submitted, and that such recordings are the property of VEST for purposes of publication, display or broadcast (print, web, digital display and all other forms of media). In addition, you understand and agree that all ownership and intellectual property rights related to the video and audio recorded by VEST belong to VEST.

Any files that are uploaded will be shared outside of the organization they belong to.
